This questionnaire is designed to assess your organization’s alignment with the requirements of the European Union’s Artificial Intelligence Act (AI Act).

Please answer each question carefully, providing supporting documentation or details where applicable.

This assessment will help determine whether your organization must comply with the AI Act and, if so, the specific obligations it must fulfill.

The EU AI Act applies fully from 2 August 2026, with earlier deadlines for specific provisions: Codes of practice by 2 May 2025 (Article 56), reporting serious incidents by 2 August 2026 (Article 73), and compliance for pre-existing high-risk AI systems and general-purpose AI models by 2027–2030 (Article 111). Key chapters start earlier, from 2 February 2025 (Article 113).
